Health Maintenance Organization

(HMO)

HMOs are the oldest form of managed care plan and they have been around and successful for a long time. Basically they usually offer a range of health benefits, including preventive care, for a monthly fee that is the same every month.

They offer co-pay on prescriptions.

There are several types of HMOs.

There is the type of HMOs offered at most jobs that is a staff or group model HMO.  They get to know you personally in many instances and they are looking at your healthcare on a long term basis.   They want to KEEP you well.

Some HMOs contract with physician groups or individual doctors who have private offices, called individual practice associations (IPAs) or networks. In this type of plan you will be given a list of doctors to choose from and will have to pick one as your primary care physician.  It is a little bit of a hassle to change your Primary Care Physician, so choose wisely.

This Primary Care Physician sees you whenever you have a health issue  -- and for yearly check-ups. Unless you are a truly unforgettable character your doctor probably won't recognize you on the street.

 If your primary care physician decides you need to see a specialist or other doctor s/he will give you a referral. You will even need a referral to go to the emergency room in some cases.  Real problems can arise over deciding on the definition of "EMERGENCY."  Insurance companies see altogether too many people leaving the emergency room with nothing needed to fix your problem but a swab of iodine and a band-aid.  You will need to discuss where the line is drawn before you sign on the dotted line.

With most HMOs you will not have to pay anything to see your primary physician for treatment, but some do have a very small co-pay of $5-$10 per visit.

In practically all of these HMO plans you must get a referral to go out of the network or you will be required to pay for that visit yourself, in full. In some cases you might even be required to pay in full if there is a network doctor available and you refuse to see that one.
